Your First Visit

This appointment is complimentary and will determine if orthodontic treatment is needed, when treatment should begin, and the approximate fees involved. One parent should accompany every child under eighteen years of age. This appointment should provide the information you need to make the decision to pursue orthodontic treatment. Records may be taken on this day (if the schedule allows) therefore combining the first and second appointments.

Second Visit: Diagnostic Records*

A complete set of diagnostic records includes digital 3-Dimensional study of models of the teeth, photos of the patient’s face and teeth, and x-rays. This information is used to develop a custom treatment plan for each patient and is helpful in tracking the progress of treatment as the teeth move under orthodontic care.

Please help us be prepared for your first appointment by completing this Patient Information and Medical History form. Download the form below (in either Word or PDF format) to your computer, print it out, complete the form, and bring it with you to your first appointment. If you're unable to read PDF files, you can download Acrobat Reader free from Adobe.
Consultation

Dr. Wainwright may want to schedule a consultation appointment for some patients. This gives him time to measure and study the diagnostic records before deciding your particular treatment plan.

Appointments

Initial phases of treatment (bonding braces or installing other appliances) require longer appointments (thirty minutes or longer). These appointments are most often scheduled during the morning hours. Once active treatment is underway, most adjustment appointments will take approximately twenty to thirty minutes of chair time, scheduled at three- to twelve-week intervals, depending on the stage of treatment. Every effort will be made to give adjustment appointments conveniently after school or work. This time is a premium time in an orthodontic office. If an appointment must be broken due to illness, accident, etc., our office needs to be informed as soon as possible to reserve you another appointment without too many calendar days being lost. Late arrival for appointments may necessitate reappointment, or a shorter adjustment check on that day, which may delay treatment as well.

Cost

Cost of treatment depends on individual needs and will be discussed at the initial visit. We accept all insurance assignments and offer several payment options including payment by Visa, MasterCard, Discover and American Express. We work hard to make orthodontic treatment affordable.
